Neck pain can sometimes be more than just stiffness or muscle tension. If pain, tingling, numbness or weakness is travelling from your neck into your shoulder, arm or hand, you may be experiencing cervical nerve root irritation, often referred to as radicular pain.
This condition occurs when one of the nerves leaving the cervical spine (neck) becomes irritated or compressed. Everyday activities such as driving, working at a computer, lifting, or even sleeping awkwardly may aggravate symptoms.
At Belconnen Physiotherapy Clinic, we regularly assess and manage people experiencing neck pain and cervical nerve irritation from across Belconnen, Bruce, Gungahlin, Kaleen, Hawker and the wider Canberra region. A thorough assessment can help identify the source of your symptoms and guide an individualised treatment plan.
The cervical spine contains seven vertebrae with nerve roots exiting between each level. These nerves travel into the shoulders, arms and hands, supplying sensation and muscle function.
When a nerve root becomes irritated or compressed, it may produce symptoms beyond the neck itself. This is known as cervical radicular pain or cervical radiculopathy.
Common causes include:
·Disc bulges or disc herniation
·Age-related joint or disc changes
·Narrowing of the openings where nerves exit the spine (foraminal narrowing)
·Neck injuries such as whiplash
·Inflammation around the nerve root
·Repetitive or sustained neck postures
While the symptoms can be concerning, many people improve with appropriate conservative management.
Symptoms vary depending on which nerve is affected but may include:
·Neck pain
·Pain travelling into the shoulder or arm
·Tingling or “pins and needles”
·Numbness in the arm or fingers
·Muscle weakness
·Burning or sharp shooting pain
·Pain that increases when looking up or turning the head
Some people notice symptoms only occasionally, while others experience discomfort throughout the day.
Because several conditions can produce similar symptoms, an accurate assessment is important.
Several factors may contribute to cervical nerve irritation.
A cervical disc may bulge or herniate, placing pressure on a nearby nerve root.
As we age, the discs and joints naturally change. In some people these changes reduce the available space around spinal nerves.
Falls, sporting injuries, motor vehicle accidents or sudden movements may irritate the cervical spine and surrounding structures.
Long periods at a desk, frequent device use or sustained awkward positions may contribute to neck pain and aggravate sensitive nerves.
A physiotherapist will begin with a detailed history and physical examination.
This typically includes assessing:
·Neck movement
·Arm symptoms
·Muscle strength
·Reflexes
·Sensation
·Neural tension tests
·Posture and movement patterns
Imaging such as MRI or CT scans is not always required. Your healthcare provider will determine whether imaging or referral is appropriate based on your symptoms and clinical findings.
Physiotherapy aims to reduce pain, improve movement and help you return to your usual activities as comfortably as possible.

Many people improve with conservative management, including physiotherapy, activity modification and time. Surgery may only be considered in specific situations after medical assessment.
Recovery varies depending on the underlying cause, symptom severity and individual factors. Some people notice improvement over several weeks, while others require a longer rehabilitation period.
In many cases, remaining active within comfortable limits is recommended. Your physiotherapist can advise which exercises and activities are appropriate during recovery.
Seek prompt medical assessment if you experience significant arm weakness, loss of coordination, changes in bladder or bowel function, severe trauma, or rapidly worsening neurological symptoms.
Although not every neck injury can be prevented, maintaining good neck strength, mobility and posture may reduce the likelihood of recurring episodes.
Your physiotherapist may recommend:

·Stretching where appropriate
·Workstation advice
·Movement breaks during prolonged sitting
·Safe lifting techniques
·Gradual return to sport or physical activity
