A 5th metatarsal fracture is a common foot injury that affects the long bone on the outside of the foot, connecting to the little toe. These fractures can occur during sport, while walking on uneven ground, after rolling the ankle or following a fall. Depending on the location and severity of the fracture, treatment may involve a walking boot, cast or, in some cases, surgery.

The fifth metatarsal is the outermost long bone of the foot. Fractures can occur in different parts of the bone, and the type of fracture often influences treatment.
Common types include:
Avulsion fractures – where a small piece of bone is pulled away by a tendon or ligament.
Jones fractures – occurring further along the bone in an area with a relatively limited blood supply, which may take longer to heal.
Stress fractures – developing gradually from repetitive loading rather than a single injury.
An accurate diagnosis using clinical assessment and medical imaging helps determine the most appropriate treatment plan.
A fracture may occur due to:
Rolling the ankle
Sporting injuries involving twisting or sudden direction changes
Falls or awkward landings
Repetitive running or jumping
Increased training loads without adequate recovery
Active individuals, runners and athletes may be more susceptible to stress-related fractures.
Symptoms vary depending on the type of fracture but often include:
Pain on the outside of the foot
Swelling and bruising
Difficulty walking or bearing weight
Tenderness when touching the injured area
Pain that increases during activity
If you are unable to bear weight after an injury or your pain is severe, prompt medical assessment is recommended.
Once your fracture has been assessed and managed by your GP or orthopaedic team where required, physiotherapy plays an important role in rehabilitation.

After a period of immobilisation, the foot and ankle often become stiff. Gentle exercises and manual therapy, where appropriate, may help improve movement.
Muscles around the foot, ankle and lower leg commonly weaken during recovery. A progressive strengthening program helps improve function and confidence.
Following a foot fracture, balance and proprioception can be affected. Targeted exercises help retrain stability and reduce the risk of future injuries.
Your rehabilitation program is progressed according to your symptoms, healing and individual goals. Higher-level exercises are introduced gradually to prepare for work, running or sport.
Healing times vary depending on the type of fracture, your overall health and your treatment. Many fractures require several weeks of protection before gradually increasing activity. Your treating medical team and physiotherapist can provide advice based on your individual recovery.
Some fractures allow protected weight bearing in a walking boot, while others require a period of non-weight bearing. Always follow the advice provided by your treating healthcare professional.
Not everyone requires surgery. Some fractures heal well with conservative management, while others—particularly certain Jones fractures or displaced fractures—may require surgical treatment. An orthopaedic surgeon will determine whether surgery is appropriate.
Returning too early may increase the risk of ongoing pain or reinjury. Your physiotherapist will guide you through a gradual progression based on your healing, strength, mobility and functional testing.
